Design a medication log printable report for clients. The report would be specific to one client. It would use the Medications in ChildTrax for the month and year selected. The log will be used for:
- Reminds the care giver to administer medications.
- Monitors when the client takes medications.
- Reports medication adherence.
- This log will be filled out by agency staff and/or foster parents. The log should have a field for Date Administered, Time Administered Amount of Medication Administered, Comments/Reactions note field. At the top of the log show all current medications.
Add to medication screen:
- Checkbox for Prescription/Non-Prescription on medication screen.
- Checkbox for Refrigeration Required on medication screen.
- Change Dosage to Dosage Amount.
- Add Times to administer on medications screen.
- Add Start Date of Administration and change the End Date to End Date of Administration on the medication screen.
- Add a field for Special Instructions to the medication screen.
- Add a field to list who gave permission to administer medication to client on medication screen.
- Add medication allergy field.
- Add dynamic field for all of the above.
